Job Description

The focus of care management is identifying and managing patient outcomes across the continuum. The Manager of Care Management is a strategic leader overseeing the care management function within our healthcare organization. Their role is to ensure that patient care coordination, care management, and related services are delivered effectively, efficiently, and in compliance with regulatory and quality standards.

The Manager of Care Management leads and optimizes the care management processes so that patients receive timely, appropriate, and integrated care across all settings. This involves aligning clinical, administrative, and operational functions to improve health outcomes, reduce avoidable hospitalizations, and enhance patient satisfaction.

Duties include but are not limited to:

  • Collaborate with interdisciplinary care teams to coordinate patient care, develop care plans, and support positive patient outcomes across the continuum of care.
  • Conduct comprehensive patient assessments, including physical, psychosocial, environmental, financial, and support system evaluations.
  • Facilitate discharge planning, communication among providers, and coordination with community resources, patients, and families.
  • Manage utilization review activities, including medical necessity reviews, payer communication, length-of-stay monitoring, and resource utilization.
  • Serve as a clinical resource and advocate for patients, families, and staff by providing education, consultation, and support for complex cases.
  • Participate in quality improvement initiatives, staff development, and ongoing professional education while promoting organizational values and positive team relationships.
  • Maintain compliance with regulatory standards, licensure requirements, and Minnesota Department of Health Medical Home Certification expectations.

 

Hours/Shifts
80 hours biweekly, Monday-Friday. 


Compensation
This is a salaried-exempt position with a pay range of $89,772.80-$120,640 annually. Please note that final compensation will be determined based on experience, qualifications and internal equity considerations.

  1. Education/Special Skills: Graduate of a school of nursing. Bachelor of science/art in nursing preferred. Current Minnesota nursing license required. This position requires current Healthcare Provider level CPR certification upon hire or must be obtained within 6 months.
  2. Experience: Care Management experience preferred. Three years’ clinical experience in a healthcare setting is preferred. 
  3. Job Knowledge: Demonstrates sound clinical skills. May have an identified specialty of practice but must be able to function in all areas of care management to ensure continuity of care in the absence of colleagues. Demonstrates customer-focused interpersonal abilities to interact effectively with physicians, health team members, community agencies, and patients/families with diverse opinions, values, and religious/cultural ideals. Ability to work autonomously with little direction, oversee a team of multi-disciplinary health care professionals and be directly accountable for this practice. Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, facilitating, problem solving, decision making, priority setting, work delegation, and work organization.
  4. Care Management core competencies include: Knowledge of the role of care management in assuring patient care across multiple settings. Knowledge of appropriate interventional strategies to reduce the length of stay and manage utilization of resources, Knowledge of reimbursement mechanisms and managed care issues. Knowledge of community resources and referral mechanisms to assure coordination of discharge planning. Knowledge of Quality Improvement processes.
  5. Successful completion of the pre-employment health screening process required.
  6. Must be able to attend the work site according to the schedule.
