Position Summary
The Adult Mental Health Case Manager supports continuity of care for adults receiving behavioral health services across Hampton and Newport News. This position coordinates services that help clients stabilize, meet essential needs, and progress toward treatment goals.
Key responsibilities include completing assessments, developing and updating treatment plans, providing advocacy and supportive counseling, coordinating community resources, documenting services, and responding to urgent situations with appropriate crisis intervention.
Required qualifications:
- Bachelor’s degree in a Human Services field (e.g., Social Work, Psychology, Sociology, Counseling)
- One (1) year of case management experience
Your Typical Day
Your day as an Adult Mental Health Case Manager is structured around a Monday–Friday, 8:00 a.m.–5:00 p.m. schedule, balancing planned client contacts with time for coordination and follow-through. You may start by reviewing updates and prioritizing needs, then connect with clients in the community and by phone to ensure supports are in place. Throughout the day, you collaborate with providers and partner agencies, confirm appointments and services, and complete timely notes so the care team stays aligned. When unexpected concerns arise, you shift focus quickly, communicate clearly, and help connect clients to immediate supports.
