Job Description:

Liaises between Care Management, Nursing, and Hospital providers to solve patient challenges. Incorporates patient and family care goals into transition planning. Connects with hospital providers to review transition plans. Follows up on diagnostic test results to ensure availability for decision-making. Protects patient privacy in all interactions. Educates nursing and care management teams. Identifies patients with discharge delays and facilitates safe discharges. Manages interdisciplinary patient transition plans. Escalates potential denials and documents avoidable days. Participates in length of stay meetings. Maintains knowledge of federal policies. Participates in regulatory preparations. Other duties as assigned.

• Computer skills that include Microsoft applications and Hospital Electronic Medical Record (EMR) [Required]
• Case Management training [Preferred]
• Specific knowledge and skills delineated by the unit of work (see competencies) [Preferred]

Education:
• Bachelor's of Nursing [Required]

Work Experience:
• 2+ years experience [Required]
• Experience in Care Management, Managed Care, Discharge Planning and Utilization Management [Preferred]
• Prior leadership experience [Preferred]

Licenses and Certifications:
• Registered Nurse (RN) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]

Pay Range:

$31.53 - $52.24
