As an Evaluator, you will conduct comprehensive assessments and evaluations for individuals seeking behavioral health services while helping determine appropriate treatment recommendations and levels of care. This role is responsible for establishing provisional diagnoses, developing treatment plans, coordinating referrals, and supporting continuity of care across programs and services. Evaluators may also provide crisis intervention, safety planning, and brief therapeutic support as needed. Key Responsibilities:
- Conduct screenings, assessments, and evaluations to determine eligibility and appropriateness for behavioral health services
- Establish provisional diagnoses and develop individualized treatment recommendations and referral plans
- Coordinate follow-up services and referrals to appropriate internal and external programs
- Provide crisis intervention, safety planning, and brief therapeutic support as needed
- Maintain timely, accurate, and compliant clinical documentation and data collection activities
- Meet department-specific collaborative service goals and productivity expectations
- Participate in peer review staffing, departmental meetings, and staff development activities
- Remain knowledgeable regarding program admission criteria, referral processes, and community resources
- Support quality assurance, compliance, revenue cycle, and documentation standards
- Deliver educational presentations and mental health awareness information within community settings as needed
- Assist with program evaluation activities, data collection, and grant-related initiatives
- Report critical incidents and communicate concerns appropriately to leadership
- Complete all required trainings, evaluations, and compliance requirements
- Perform additional duties and special projects as assigned by leadership
Education and/or Experience Qualifications:
- Master’s degree in Social Work, Psychology, Counseling, or related behavioral health field required
- State licensure appropriate to work location and area of practice required
- Previous experience conducting behavioral health assessments, evaluations, or crisis intervention preferred
- Knowledge of behavioral health treatment planning, referral coordination, and diagnostic assessment practices preferred
Required License/Certification:
- Current unrestricted state licensure appropriate to work location and scope of practice required
- Current driver’s license, acceptable driving record, and current auto insurance required
Additional Qualifications:
- Strong clinical assessment, interviewing, and treatment planning skills
- Excellent verbal and written communication abilities
- Ability to communicate effectively with individuals from diverse backgrounds and levels of need
- Knowledge of behavioral health programs, referral processes, and community resources
- Strong organizational, documentation, and time management skills
- Ability to manage crisis situations calmly and effectively
- Ability to apply critical thinking and sound clinical judgment in decision-making
- Proficiency with electronic health record systems and standard office software
- Commitment to ethical standards, confidentiality, and person-centered care

Employment Requirements:
- Successful completion of background checks including criminal record, driving record, abuse/neglect, and fingerprint checks
- Completion of New Hire Orientation upon hire
- Completion of all required trainings, including Relias training, upon hire and annually thereafter
- Maintenance of unrestricted licensure for work location and scope of practice
- Current driver’s license, acceptable driving record, and current auto insurance required
Physical Requirements:
ADA Consideration – Sedentary Work:
Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including repetitive movements of hands, fingers, and arms for typing and documentation tasks.
Sedentary work primarily involves sitting but may include occasional walking or standing for brief periods throughout the workday.
