Social Worker/Executive, Communities Care (contract till 31 Dec 2026)

KEY RESPONSIBILITITES

• Support care integration work in Toa Payoh subregion with partner organisations, including SCC, AAC, HPC+, and HT providers, and to achieve the deliverables specified for the Integrated Community Care Provider.
• Work with ground teams to implement the sub-regional workplan, and monitor and report on the progress of the workplan.
• Coordinate and facilitate regular meetings and check-ins for partner organisations to build understanding and work through implementation and other challenges.
• Engage and build networks and partnerships with other community care providers, regional health systems, grassroot organisations and other stakeholders.
• Assess resources and identify opportunities for care integration and coordination collaborations across ICCP partners and with external partners
• Support in the development and implementation of joint referral workflow with ICCP partners to support care assessment, planning and coordination for seniors.
• Review and track the effectiveness of interagency workflows in seniors’ care.

REQUIREMENTS

• Bachelor in Social Work or relevant fields (Psychology, Community Work, Healthcare);
• At least 2-3 years of relevant working experience; Knowledge and experience in eldercare/healthcare sector will be advantageous;
• High proficiency in written and spoken English;
• Strong interpersonal and communication abilities with individuals and systems at multi-levels;
• Good facilitation skills
• Team player with an ability to function effectively in a fast-paced environment.
