Provides ongoing diabetes education for healthcare professionals and community groups as needed. Promotes DSME services through community relations activities. Works with other health professionals on diabetes management projects and programs. Maintains knowledge of current issues regarding patient education and diabetes care. Oversees the planning, implementation, and evaluation of the DSMES service. Liaises between DSMES team members, other departments, and administration. Monitors and facilitates the maintenance of DSMES team members' qualifications, including CE credits, training, competency, licensures, and registrations. Maintains ADA Recognition and participates in evaluating the DSMES service's effectiveness. Assesses patient learning needs, develops individualized teaching plans, teaches, and evaluates patient learning in outpatient settings. Plans and provides individualized and group instruction in diabetes self-management to patients and their families/significant others. Documents all aspects of the education services received by patients in a permanent education record. Assists in planning and conducting program evaluations. Other duties as assigned.Knowledge, Skills, and Abilities:

Education:
• Bachelor's [Required]
Field of Study:
• Graduate of an accredited school of higher learning.
Work Experience:
• 2+ years direct diabetes education. [Required]
• 5+ years healthcare experience [Required]

Pay Range:
