Essential Responsibilities:
Patient Care:
- Conduct comprehensive assessments of patients' health status.
- Develop, implement, and evaluate individualized care plans.
- Administer medications, treatments, and other clinical procedures as needed.
- Monitor and document patients' progress and response to treatment.
Case Management:
- Coordinate patient care activities and ensure continuity of care.
- Facilitate communication between patients, families, and healthcare providers.
- Advocate for patients' needs and assist in navigating healthcare systems.
- Provide patient education on disease management, treatment options, and preventive care.
Clinical Coordination:
- Collaborate with physicians, specialists, and other healthcare team members.
- Schedule and manage patient appointments, follow-ups, and referrals.
- Ensure compliance with healthcare regulations and clinic policies.
- Participate in quality improvement initiatives and clinical audits.
Administrative Duties:
- Maintain accurate and up-to-date medical records and case notes.
- Prepare and submit reports on patient outcomes and case management activities.
- Assist with the development and implementation of clinic protocols and procedures.
Job Qualifications
- Education:
- Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited institution.
- Current and valid RN license, allowing practice in the State of Texas.
- Experience:
- Minimum of 2 years of clinical nursing experience, preferred.
- Previous experience in case management or related field, preferred.
- Skills:
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Proficient in electronic medical records (EMR) and other healthcare software.
- Ability to manage multiple priorities and work in a fast-paced environment.
- Compassionate and patient-centered approach to care.
