What you will do:
The Supervisor will oversee and coordinate each Social Workers/Case Workers within the different service lines; ensuring appropriate workflows, panel management, focus on attributed lives, meeting benchmarks and ensuring performance measures are met. The Supervisor will work closely with the other Social Work Supervisors in the Service Line as well as the Directors of Care Coordination to ensure integrated care is provided to our patients and that Trinity Health and SPHP's Priority Triple Aim is met.
Responsibilities:
- Provides outstanding customer service in a manner reflective of the St. Peters Health Partner's Network and the Trinity Health Mission and Values.
- Builds and maintains relationships both within SPHP and the Community to support and meet our patient's needs.
- Supports and facilitates communication between network leadership and Directors accountable for transitions of care.
- Maintains an up-to-date knowledge base of SPHP organizational resources as well as Community Resources
- Collaborates to monitor and respond to complaints; Facilitates the resolution of problems and conflict by encouraging decision making by those responsible, remaining objective, respecting feelings, and soliciting perspectives to each new idea.
- Implements corrective action plans as necessary.
- Demonstrates commitment to leadership development.
- Attends and participates in leadership development sessions.
- Orients, trains, and provides ongoing staff education and development to elevate staff competency and quality of care and services.
- Promotes teamwork by keeping others informed, participating in group decision making, working to accomplish team objectives, and by soliciting feedback about effectiveness as a team member.
- Obtains and maintains competency in accessing and documenting patient information in the electronic patient medical record.
- Updates policies and procedures that guide and support the provision of services.
What you will need:
- Masters degree in Social Work, NYS LMSW/LCSW required.
- Case Management certification preferred.
- 5 years of full time experience in which clinical competence and administrative management skills have been demonstrated in a leadership role required.
- 10 years of full time direct practice experience preferred.
- Excellent verbal, written communication and interpersonal skills.
- Ability to develop, implement, and administer budgets.
- Strong leadership skills, including decision-making, coaching, teaching and counseling
- Strong analytical and critical thinking skills, including the ability to resolve complex administrative, technical or human situations, to anticipate and resolve a broad range of issues
Pay Range: $34.05- $48.89
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
