On-Call Registered Nurse (7 On/7 Off)
After-hours support. Mostly remote. Critical impact.

The After Hours On-Call Registered Nurse provides skilled nursing support, symptom management, and crisis intervention to hospice patients receiving care in home and facility settings during evenings, weekends, and after-hours coverage while collaborating closely with the interdisciplinary team to ensure compassionate, quality care.
Schedule, Hours & Territory
• Dedicated on-call role with a rotating 7-days on/7-days off schedule
• Coverage hours are 4:00 PM to 8:30 AM and the full weekend
• Ingham County, MI

Key Responsibilities
• Triage patient and caregiver calls after hours
• Provide clinical guidance and symptom management
• Complete urgent in-person visits as needed
• Coordinate care with interdisciplinary team members
• Document all assessments, interventions, and communication accurately and timely in the EMR HCHB (Homecare Homebase)
• Active Registered Nurse (RN) license
• Hospice field experience preferred
• Strong clinical judgment and decision-making skills
• Experience using HCHB (Homecare Homebase) preferred
Field Role Requirements
• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
