JOB SUMMARY
The Outpatient Program is specifically designed for people who struggle with substance use disorders, mental health issues or co-occurring disorders. The Outpatient Program works with high-risk populations from a variety of backgrounds. Our programs are open and welcoming to adult participants, of any gender identity. We accept the majority of participants who seek Outpatient treatment, including walk-in’s, referrals from other agencies and step-down transitional clients from residential treatment programs.
The Housing Case Manager II is a hybrid role that merges housing navigation with recovery-focused case management. This position supports clients across residential, outpatient, and community settings to maintain housing stability, engage in treatment, and transition successfully into independent living. Services are delivered in alignment with Drug Medi-Cal (DMC) billing requirements, including unbundled residential case management and outpatient care coordination. The Housing Case Manager II conducts field-based work with housing authorities, landlords, and community partners while collaborating closely with clinical teams to ensure continuity of care.
KEY RESPONSIBILITIES
Housing Navigation and Recovery Support
Provide individualized case management focused on housing stability and recovery.
Lead housing navigation tasks: applications, documentation, waitlist management, and landlord advocacy.
Deliver recovery supports including budgeting, life skills, relapse prevention, and community integration.
Facilitate client workshops on rental readiness, tenant rights, and housing stability.
Conduct field-based visits with landlords, housing providers, and residential programs.
Clinical Integration and Care Coordination
Embed with residential and outpatient teams to coordinate client care and support treatment engagement.
Participate in case consultations, multidisciplinary planning, and discharge planning.
Provide referrals to benefits programs (SSI/SSDI, CalFresh, GA, CalAIM ECM/CS) and community supports.
Maintain a caseload consistent with Medi-Cal and agency productivity expectations.
Documentation and Compliance
Complete progress notes, service plans, and assessments in the electronic health record (EHR) within agency and Medi-Cal timelines.
Ensure compliance with HIPAA, 42-CFR Part 2, and county contract requirements.
Support data collection, reporting, and quality improvement efforts.
Participate in program audit preparation and maintain accurate client files.
Administration and Team Collaboration
Attend supervision, team meetings, and trainings as assigned.
Collaborate with housing authorities, landlords, and county partners to expand housing options.
Assist with onboarding of new staff, interns, or peers by serving as a mentor or buddy when needed.
QUALIFICATIONS
Education, Certification, and Experience
High school diploma or GED.
Current registration with a California-accredited AOD certifying agency (CADTP, CAADE, CCAPP) required.
First Aid and CPR certification
Desired
AOD certification from an accredited certifying agency (CADTP, CAADE, CCAPP) preferred.
Minimum two years’ case management or housing navigation experience preferred.
Experience with justice-involved, unhoused, or high-barrier populations strongly preferred.
