Job Description

The Medically Assisted Treatment Case Manager provides program oversight, administrative support, and acts as point of contact with DPH for Suboxone, Naloxone, Vivitrol treatment services across CHC. The Coordinator/Case Manager will ensure that all data is collected and entered into the appropriate systems, and that follow up with patients to coordinate appointments is completed and documented. The position works in conjunction with the CMO, the ACTION & Behavioral Health Director, and the COO, and demonstrates excellent decision-making skill, problem solving, and attention to detail. The position is responsible for highly sensitive information, which requires a clear understanding of medical and behavioral health confidentiality.
