The Licensed Practical Nurse at Phoenix House plays a vital role in supporting clients' health, safety, and recovery in a treatment setting. This position helps ensure that clients receive timely medical support, accurate medication assistance, and compassionate care throughout their treatment experience. As a Licensed Practical Nurse, you are often one of the first points of contact for clients with health-related needs. Your attention to detail, care, and follow-through help create a safe, structured, and supportive environment where clients can focus on recovery and healing.

Available Monday-Friday, 6:30 AM – 10:00 AM and 4:30 PM – 8:00 PM

The Licensed Practical Nurse will perform the following tasks:
- Provide direct nursing care to clients under the supervision and direction of clinical leadership.
- Maintain patient confidentiality in accordance with Phoenix House policies and procedures.
- Oversee unlicensed assistive personnel (e.g. CNAs, Medical Assistants, and Medical Technicians).
- Perform routine nursing procedures such as EKGs, wound care, and specimen collection. Administer prescribed medications, including oral, topical, and injectable forms (IM, Sub-Q, intradermal) in accordance with NYS scope of practice and facility policies.
- Document vital signs, weight, and other clinical assessments to monitor patient condition and report significant findings to clinical leadership.
- Observe and promptly report any changes in patient behavior, appearance, or health status to the appropriate clinical leadership.
- Engage in health education and reinforce treatment plans, medication adherence, and self-care strategies.
- Other duties as assigned.

- Hourly Rate: $23.51- $35.29
REQUIRED EDUCATION
- High School Diploma or GED
- Graduate of an accredited nursing program
LICENSES/CERTIFICATIONS
- Current NYS Licensed Practical Nurse
- First Aid Training
- CPR Training
- Narcan Training
- Infection Control Certification
- Child Abuse Certification
REQUIRED EXPERIENCE
- At least three years of related experience
- Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugs

- This job operates in an indoor medical clinical environment. This role also routinely comes into contact with patients who may have contagious illnesses.
