Job Description

Position Summary

Responsible for implementation of programmatic content, coordination of activities, data collection, day-to-day operations, and management of staff for COHC’s shelter and mobile van locations. Monitor all operations to ensure that all shelter and mobile van sites remain in compliance with regulatory standards pertaining to state licensing, fire and safety codes, infection control, staffing, Joint Commission and other applicable regulatory funding agencies in conjunction with the Compliance department. Under the direction of the Chief Medical Officer or his designee, create opportunities in the community to reach and educate community clients and partners as well as potential business expansion, marketing and development opportunities. Position is also responsible for envisioning, establishing and implementing the “Connection to Care Center” model. Through model innovation, the incumbent focuses on enhancing the care delivery model to meet the healthcare needs of the homeless population while tackling areas impacting social determinants of health for the population.

Essential Position Duties

  1. Works closely with the CMO and senior leadership on developing and implementing the “Connection to Care Center” model including but not limited to model building, funding, staffing, compliance, future sustainability and expansion. Works closely with external entities to promote the “Connection to Care Center” model, identify funding and sustainability opportunities.
  2. Ensures optimal planning and coordination of shelter location and van location operations and activities.
  3. Anticipates and resolves patient related problems to ensure positive outcomes.
  4. Responsible for the day-to-day supervision of Health Care for the Homeless staff at shelter locations, and mobile van staff.
  5. Works to keep programs on schedule, within stated budgets and functioning smoothly.
  6. Oversees patient flow and problem resolution for an optimal patient centered environment for health care delivery.
  7. Generates meaningful reports to track patient volume at respective sites.
  8. Assesses staffing and coverage needs at shelter and mobile van locations and collaborate with senior management to address.
  9. Identifies areas within the shelter and mobile van sites that are in need of enhancement/improvement and provide recommendations to senior management on addressing respective needs.
  10. Participates, coordinates and maintains regular meetings to discuss program progress, opportunities, outcomes, issues, etc. with partner site management (shelters, soup kitchens, and community-based organizations).
  11. Monitors community-based facilities to ensure compliance with applicable local, state federal and managed care requirements including licensing, fire, safety and infection control, as well as applicable accreditation standards including Joint Commission. Adheres to licensing time sensitive deadlines.
  12. Monitors cash received from self-pays/co-pays at mobile van sites and delivers cash to the Finance Department.
  13. Ensures supplies are ordered, delivered and received at the mobile van and shelter sites according to site specific needs and health center protocol.
  14. Participate in National Health Care for the Homeless Council Meetings and conferences.
  15. Implements and monitors logs, forms, protocols, procedures and competencies as needed and/or requested to ensure adequate documentation, tracking, and performance outcomes.
  16. Responsible for monitoring patient productivity and creating/monitoring reports.
  17. Performs miscellaneous duties as required by senior management.
  18. Attend weekly/monthly staff meetings.
  19. Must maintain appropriate attendance.

 

Compliance Responsibilities

  • In collaboration with management, assists with ensuring quality and regulatory compliance within the nursing department and organizationally as appropriate.
  • Complies with applicable legal requirements, standards, policies and procedures including but not limited those within the Compliance Process, Code of Conduct, and HIPAA.
  • Participates in required orientation and training programs, as required.
  • Reports concerns and suspected incidences of non-compliance in accordance with COHC Compliance Reporting Process.
  • Cooperates with monitoring and audit functions and investigations.
  • Participates, as requested, in process improvement responsibilities.

POSITION QUALIFICATIONS

 

Core Competencies/Skill Sets

  • At least two years of healthcare, community health center or social services supervisory experience
  • Strong verbal, analytical, organizational and interpersonal skills required
  • Proficient in Microsoft Office and familiar with the use of electronic medical records.
  • Must be a team player and able to work in collaboration with others
  • Demonstrates strong customer service skills

 

 

Professional Experience/Educational Requirements

  • At least two years of healthcare, community health center or social services supervisory experience
  • A minimum of a Bachelor’s  Degree required.
  • Valid Connecticut driver’s license

Certification/Licensure

  • Valid Connecticut Driver’s License and transportation required
