
I/DD Case Manager (54241)
Job Description
POSITION SUMMARY:
Case management positions consist of advocating and assisting participants with waiver services.
DUTIES AND RESPONSIBILITIES:
- Conduct annual assessment and person-centered service plan
- Conduct monthly face to face visits with participants
- Assist clients with accessing community resources
- Monitor participants progress with waiver services including health, safety and welfare in monthly contact note
- Assist with building a network of supports
- Assure participant choice and access to community resources.
- Return phone calls and emails promptly.
- Complete and submit documentation by required deadlines.
- Perform other related work as required and assigned.
QUALIFICATIONS:
BA degree in human services: psychology, social work, sociology, special education or other related behavioral sciences. One year of experience working with ID/DD individuals (ex: volunteering or having a relationship with family or friend who may have ID/DD). OR one year of case management experience.
Valid Driver’s license and vehicle insurance required

JOB REQUIREMENTS / EXPECTATIONS:
- This job consists of sitting to complete necessary paperwork on computer
- Must be able to operate a motor vehicle and have reliable transportation with valid insurance
- Must provide verbal and communication with team members
- Job setting is at the corporate office and/or home office setting
- Documentation is time sensitive and must follow waiver regulations
- Work schedule is Monday-Friday 8am-5pm. Schedules can vary depending on participant visits. Flexible working hours.
