Job Description

 

POSITION SUMMARY:

Case management positions consist of advocating and assisting participants with waiver services.

DUTIES AND RESPONSIBILITIES:

  • Conduct annual assessment and person-centered service plan
  • Conduct monthly face to face visits with participants
  • Assist clients with accessing community resources
  • Monitor participants progress with waiver services including health, safety and welfare in monthly contact note
  • Assist with building a network of supports
  • Assure participant choice and access to community resources.
  • Return phone calls and emails promptly.
  • Complete and submit documentation by required deadlines.
  • Perform other related work as required and assigned.

QUALIFICATIONS:

BA degree in human services: psychology, social work, sociology, special education or other related behavioral sciences. One year of experience working with ID/DD individuals (ex: volunteering or having a relationship with family or friend who may have ID/DD). OR one year of case management experience.

Valid Driver’s license and vehicle insurance required

  • Ability to advocate for the needs of participant

JOB REQUIREMENTS / EXPECTATIONS:

  • This job consists of sitting to complete necessary paperwork on computer
  • Must be able to operate a motor vehicle and have reliable transportation with valid insurance
  • Must provide verbal and communication with team members
  • Job setting is at the corporate office and/or home office setting
  • Documentation is time sensitive and must follow waiver regulations
  • Work schedule is Monday-Friday 8am-5pm. Schedules can vary depending on participant visits. Flexible working hours.
