Salary Range
The expected base rate for this Glens Falls, New York, United States-based position is $28.00 to $35.06 per hour. (Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements).

As a Licensed Practical Nurse, you will be an important member of the patient care team. You will utilize the nursing process for administering care of both well patients and those with health problems in an outpatient setting. The Physician Practice team works with community agencies and resources in order to promote, restore, and maintain optimal health for patients and families in the surrounding area.
How You Will Fulfill Your Potential (Responsibilities)
- Documentation of all patient care is required
- Prepare patient by collecting and sharing data; blood pressure, pulse, respiratory rate, height/weight, Date of LMP, general history and chief complaint and identifies need for medication refills.
- Collection of JCAHO required data elements, such as domestic violence, pain, and forms completion.
- Assists and/or prepares patients for surgical procedures, tests, treatments and other diagnostic or therapeutic procedures.
- Assists the health care provider in determining current and potential needs and /or problems with the patient.
- Document all patient assessment elements obtained.
- Demonstrates awareness of risk factors associated with: age, size, sensory perception, motor function, and medications.
- Assists in the evaluation of supplies, equipment, and facilities.
What You Bring (Qualifications)
Education/Accredited Programs: Graduation from a Licensed Practical Nursing Program
Licenses/Certificates/Registrations:
Current license and registration to practice Practical Nursing in New York State
Infection Control
BLS Certification (can be obtained at time of hire)
Skills/Abilities:
Ability to complete basic mathematical calculations and complete tasks in efficient/timely manner
Ability to provide required nursing care under the direction of the medical provider
Ability to communicate/work effectively, verbally and in writing with personnel, patients, and medical providers
Ability to recognize patient care problems and to react appropriately in patient care emergencies utilizing nursing resources
Responsibility for nursing self-development and keeping current with advances in nursing knowledge and techniques
