Job Description

DUTIES:
1. Provide triage and intake to clients seeking Ontario Structured Psychotherapy (OSP) services.
2. Conduct clinical assessments (e.g. Patient Health Questionnaire-9 (PHQ-9), General Anxiety Disorder-7 (GAD-7)) to identify symptoms experienced by clients and to create individualized treatment plans.
3. Provide individual and group protocol-based structured psychotherapy, including Cognitive Behavioural Therapy (CBT) and Cognitive Processing Therapy (CPT), in adherence with the OSP Clinical Model and Training guidelines to clients with mood, anxiety, concurrent disorders and trauma experiences.
4. Provide psychotherapy services virtually and in-person based on client and program needs.
5. Participate in the OSP training program, consultation model, and ongoing education.
6. Participate in regular clinical consultation and related activities in adherence with fidelity monitoring requirements and program guidelines.
7. Maintain accurate and timely documentation and reports in adherence with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) and program specific requirements.
8. Collaborate with team members across the OSP program’s network of organizations under the provision of care.
9. Contribute to ongoing program development, program evaluation and implement new practices as required.
10. Assist in providing service navigation for clients whose needs are beyond the offerings of the OSP program.
11. Work in partnership with clients, their formal and informal supports and the team to support each clients’ successful integration into their communities.
12. Contribute to program development, research and educational activities of the service.
13. Maintain up-to-date charting/records in compliance with HSN policies and OSP procedures, and mandatory reporting requirements and standards.
14. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
15. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
16. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
17. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
18. Educate and promote health, safety and wellness in the work place.
19. Represent the department or program on various committees and in meetings as required.
20. Perform other duties as required.
