Job Description

The Care Team Hospice is seeking a compassionate and dedicated Hospice Chaplain to join our team. This is a meaningful opportunity for someone who is called to serve others through spiritual care, emotional support, and compassionate presence during life's most important moments.

As a Hospice Chaplain, you will provide spiritual guidance and support to patients, families, and caregivers navigating the end-of-life journey. Working closely with the interdisciplinary team, you'll help address spiritual needs, offer comfort during times of grief and uncertainty, and support individuals of all faiths, beliefs, and backgrounds with dignity and respect.

Schedule, Territory & Coverage:

• Part-Time Position
• Territory: Greater Indianapolis 
 

 

Key Responsibilities:

• Conduct spiritual assessments and develop individualized plans to address patient and family spiritual needs.

• Provide spiritual counseling, emotional support, prayer, and compassionate presence to patients, families, and caregivers.

• Collaborate with the interdisciplinary team to support holistic patient care and participate in care planning discussions.

• Provide grief support, bereavement resources, and spiritual guidance throughout the hospice journey.

• Maintain timely documentation and ensure services are provided in accordance with hospice regulations and patient-centered care practices.

Qualifications:

• Bachelor's degree in Divinity, Theology, Pastoral Care, Religious Studies, Counseling, Human Services, or a related field required; Master of Divinity (M.Div.) or related graduate degree preferred.

• Previous hospice, palliative care, pastoral care, ministry, healthcare, or bereavement experience preferred.

• Completion of Clinical Pastoral Education (CPE) and/or hospice-specific spiritual care training preferred.
