Job Description
The Coastal Team works with clients any families who are registered with the following GP surgeries; St Agnes, Perranporth, Carnon Downs and Chacewater.
We have regular weekly case discussion meetings which bring the team together for reflective practice and departmental updates. We work closely with our health colleagues to promote integration and understand the benefits of this approach to get the best outcomes for the people we work with.

The Coastal Team covers a rural location and access to a vehicle is essential.
