We are hiring in Cook County. This role requires frequent travel. You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times).
Key Responsibilities
Conduct home visits to assess and create personalized care plans.
Train and support caregivers, ensuring they can meet their loved ones' needs.
Monitor and report on clients’ health, coordinating with their primary care physicians, community resources, and more.
Facilitate smooth transitions and ongoing care for clients.
Maintain compliance with all relevant regulations and standards.
Full-time hybrid position offering autonomy while utilizing your assessment, care planning and case management skills.
