Position: School Based Therapist II Job Code:  

Reports to: Behavioral Health Leadership Date:

POSITION SUMMARY

Associate Licensed clinician working independently within the School system and outpatient department to conduct mental health and substance use screenings, assessments, intakes, develop individualized treatment plans, facilitate individual, family, and group therapy, case management, crisis intervention, patient education, referrals, aftercare planning, discharge, and record keeping.

 

Essential Position Duties:

  1. Develop and maintain relationships with school staff, attend any scheduled meetings, coordinate referral process.
  2. Respond to referrals timely.
  3. Attend school functions as applicable.
  4. Provide patient screenings and intake assessments as assigned in a timely manner.
  5. Utilize the most current DSM to properly diagnosis clients and update as applicable.
  6. Complete Initial and ongoing treatment/care plans for each assigned client.
  7. Maintain a caseload/panel spreadsheet to organize sessions, upcoming appointments and client engagement activities.
  8. Provide individual, family, and group therapy utilizing evidenced based practice techniques.
  9. Complete all documentation timely, including any contact with clients as applicable.
  10. Complete crisis interventions with BH clients and patients within COHC system as applicable.
  11. Provide referrals to other levels of care or needed services.
  12. Provide aftercare planning throughout treatment.
  13. Complete discharges for clients that are no longer an active client per policies.
  14. Follow BH policies and procedures.
  15. Attend any assigned meetings, supervision, or other activities.
  16. Complete Peer review audits as assigned.
  17. Performs miscellaneous duties as required by management.
  18. Must maintain appropriate attendance.

 

Additional Responsibilities

May be required to travel to other COHC sites to provide services.

May be a Hybrid position, providing services at multiple sites.

 

 

 

Compliance Responsibilities

  • Complies with applicable legal requirements, standards, policies and procedures including but not limited those within the Compliance Process, Code of Conduct, HIPAA and Corporate Integrity Agreement (CIA).
  • Follow scope of practice related to individual training and licensure
  • Participates in required orientation and training programs, as required.
  • Reports concerns and suspected incidences of non-compliance in accordance with COHC Compliance Re porting Process.
  • Cooperates with monitoring and audit functions and investigations.
  • Participates, as requested, in process improvement responsibilities.

Professional Experience/Educational Requirements

  • Master’s Degree and Associate licensure in a related field with Independent License within two years of hire.

 

 

Certification/Licensure

  • Licensed to practice in Connecticut
