Community Integration Specialist

The Community Integration Specialist supports Day Habilitation Services by assisting with staff supervision, service coordination, and person-centered programming for individuals with intellectual and developmental disabilities. This role helps develop community-based opportunities, monitors individualized service plans, coordinates transportation and supports, and ensures compliance with agency and regulatory standards. The Community Integration Specialist collaborates with families, interdisciplinary teams, and community partners to promote independence, inclusion, and meaningful engagement in both program and community settings.
